What agreat question! When doing a remote session on a client I will often do it with them while on the phone. I lead them trough a meditation to relax them and sort of talk them through where, what and how I am using the energy to treat them. I also can at that time, or at the end, verbally discuss anything that I saw or felt during the session. This can be really wonderful for a lot of my clients who are also intuitives and interested in energy work. I start most of my readings in this fashion.
Some of my clients don't want to be on the phone and I usually just ask them what time they will be laying down for bed. Then I do my work at that time it's a great addition to my evening meditation practice. Actually it sort of forces me to have an evening meditation practice.
For anyone paying me to perform service for them, I think it's a great idea to let them know when that service is being performed.
They do not have to be laying down or even relaxed for the treatment to be succesful. However I think it's a good idea. Besides it often gives the person to lay down for a bit and de-stress and that is always a good idea!

For friends & clients who I have permission to work on at any time for any reason. I just do it whenever. However at the beginning of the session I am always diligent about making sure that seems or feels like the right time.
For example, one time in a Reiki 2 class we went to work on Tere M. and every single person in the class sort of stopped working on her two minutes into the remote session. They were like, "what was her problem?" Turns out at that specific time she was not open because she was in a very tough meeting and couldn't have herself pulled from focus. Every person who tried to connect with her got a big "NO" this is not right. It was a great confirmation for the group. Luckily I have a hand full of people with open permission so we moved on to the next on the list.
